This will load the Stress Assessment questionnaire. Work through the questions answering as honestly as possible. The answers will be totaled by the reviewing pharmacist and used to give an assessment of stress levels as well as recommendations for a Metagenics therapeutic product.

This link will lead you to a pdf document which can be printed off and your answers recorded, preferably in ink. The finished quiz can either be scanned and sent to us via e-mail, or simply dropped off at the pharmacy. We will review the questionnaire using our master scoring sheet and at a follow up meeting be able to recommend some treatment options. This will be done by a clinical pharmacist who may also ask questions about your health and any other medications you may be taking. If you are currently a patient with our pharmacy, much of the information will be available on your patient file.
If you have any problems please contact the referring pharmacist who has recommended this assessment. It is important to follow up with the practitioner to further determine what treatment options might best help to reduce stress and to minimize the risk of stress related disease.
