How long is a prescription valid once it is first filled, assuming it has additional refills?

A prescription can be honoured for a full 18 months from the time it is originally filled. Even though a prescription may have refills still indicated on the prescription label, the "legal" and "usable" lifetime of that prescription is 18 months.
